The onset of the effect can be expected within 4 weeks of treatment. Treatment with Remotivshould be continued according to patient presentation, with reevaluation every 6 weeks.

Contains approximately 120 milligrams lactose. According to a systematic review, up to 12-15 grams may be tolerated by those with lactose intolerance or malabsorption, with a minimum of 15 grams required for diagnosis. Therefore, 120mg is a very low amount of lactose. However, those with the rare hereditary conditions of galactose intolerance, lactase deficiency or glucose-galactose malabsorption should not take lactose containing medications.
St John's wort extracts containing hyperforin have effects on different pharmacokinetic mechanisms. Simultaneous effects on intestinal drug efflux transporters plus CYP3A4 and other enzymes in the liver gives the herb the potential to decrease the plasma concentration of many drugs. However, extracts of St John's wort that are low in hyperforin, such as Remotiv, do not show clinically relevant interactions with efflux transporters or CYP3A4. St. John's wort may contribute to serotonergic effects when combined with anti-depressants, such as selective serotonin reuptake inhibitors.
Additionally, as a precaution, women using oral contraceptives should take additional contraceptive measures. Remotiv is contraindicated in severe depression and with the concomitant use of certain immunosuppressants, anti-retroviral HIV-drugs from the group of non-nucleoside reverse transcriptase inhibitors, protease inhibitors, certain cytostatics, oral anti-coagulants (e.g. warfarin), factor Xa-inhibitors and thrombin inhibitors. Check all potential drug interactions before prescribing St John's wort.
Discontinue Remotiv five days before any surgery.
Safe doses in pregnant women have not been determined and the herb is not recommended for use during pregnancy. Safety in lactation remains to be established.
